Billing and Insurance

At Endo Surgi Center of Old Bridge, we are committed to giving the highest quality of care and excellent service to our patients and their families.  Our Patient Business Service module is available to assist you in understanding the billing process, keeping you informed about charges for services and your obligation.  

We maintain a close working relationship with most local insurance plans.   It is important that you contact your insurance company prior to receiving services.  Each plan is different, and some provide different levels of coverage which could have an impact on your out-of-pocket costs.

Important Billing Information:

You may receive a bill from your Physician, Anesthesia and Pathology. They bill separately.

Physicians who provide service in the facility may or may not participate with the same health plans as the Facility. Patients should check with the physician arranging for the Facility service to determine the health benefits in which the physicians participate. Patients should contact their carrier for further consultation on those costs.

Your insurance company may require a Prior Authorization or referral prior to service. Please consult with your physician to ensure authorization is obtained. Failure to obtain a referral may delay your care.

You have an obligation to pay the co-payment, co-insurance and any outstanding deductibles in accordance with your insurance carrier. This amount is collected at time of service.  Alternative arrangements such as a payment plan can be made.  Please contact our billing company, Amblitel at 855-346-4357, extension 115.

Physician, Anesthesia and Pathology services provided in the Facility are not included in the Facility’s charges.

When you check in for your procedure, we will ask you to sign an “assignment of benefits form,” or ABN, to authorize payment directly to this facility. We will file a claim to your insurance carrier after your procedure.  If the insurance company sends payment to your home address, it is your responsibility to send the payment onward to the Center.

As per the Out-of-Network Consumer Protection, Transparency, Cost Containment and Accountability Act, it is your right as a patient to request information or have questions answered regarding fee or payment policy.

Facility Standard Charges are available for services provided.
